After many wonderful years of caring for your children and being a part of this community, we will be retiring and have made the very difficult decision to close our office, effective June 30, 2025.

 

What You Need to Know:

Last Day of Appointments: We will see patients for well child exams, illnesses, injuries and prescription refills through June 30th. Please contact us soon if your child needs an appointment before we close.

Sports, camp or college forms should be submitted ASAP, before June 30th. 

 

Medical Records: Your child’s medical records are confidential and will remain protected. You may request a copy of your records or have them transferred to a new provider by contacting our office. An authorization for transfer of records can be found under the “forms” tab at the top of this page. 

 

Record requests can be submitted via email to generalemailderrypeds@gmail.com

 

For patients on prescription medication it is imperative that care is established with a new physician as soon as possible because our office will be unable to fill prescriptions after June 30, 2025.
